Data Processing Consent

I am hereby granting the company Jemné Zrození s.r.o. based in Dolní Líštná 108, 73961 Třinec, identification number: 04277848, entered in the Commercial Register maintained by the Regional Court in Ostrava, Section C, file No. 63034, email address: info@jemnezrozeni.cz (hereinafter referred to as the “Controller”), consent to personal data processing under the following conditions:

When signing up for a course

  • estimated due date
  • By signing up for a course and paying its price, you consent to photographs and videos being made during the event with you, as an event participant, in them and to them being used on our website and marketing materials. The photographs and videos will not contain any of .your personal information, unless we explicitly agree on that.

When purchasing from the online shop:

  • name and surname,
  • mailing address,
  • email address,
  • phone number

When signing up for a newsletter:

  • email address,

When recruiting instructors:

  • name and surname,
  • mailing address,
  • email address,
  • phone number
  • The purpose of personal data processing is:
  • Name and surname, mailing address, email address – issuing and delivering an invoice – Courses, Online shop, instructor recruitment
  • Phone number – organizational matters regarding the course, sending out consignments – Courses, Online shop, instructor recruitment
  • Email address – sending out commercial offers and information from the Controller – Courses, Online shop, newsletter subscription, instructor recruitment
  • Estimated due date – course organization
  • The data processing period is:
  • Name and surname, mailing address – while the contractual relation lasts and 10 additional years for the reason of storing and recording invoices for accounting purposes, instructor recruitment
  • Email address – for five years from the moment of consent
  • Phone number – while the contractual relation lasts, instructor recruitment – for 5 years
  • Estimated due date – while the contractual relation lasts
  • Consent withdrawal

You are granting explicit consent with the above stated. The consent can be withdrawn at any time by sending an email to the company’s address.

  • Personal data can be provided to the following third parties (processors):
  • The instructor leading the course you signed up for.
  • The administrator/programmer of the Jemné Zrození s.r.o. website
  • External accounting company of Jemné Zrození s.r.o.

Please, note that according to the Act on Personal Data Protection and section 13 of Regulation (EU) 2016/679 of the European Parliament and of the Council (GDPR):

  • you have the right to withdraw your consent at any time,
  • you have the right to request information on how your personal data is processed by the Controller,
  • you have the right to request explanation regarding processing of your personal data by the Controller,
  • you have the right to request access to the personal data processed by the Controller and have them updated or corrected, or limit their processing,
  • you have the right to demand your personal data to be deleted,
  • you have the right to object to processing,
  • you have the right to request transfer of your data to another Controller,
  • you have the right, in case of doubt related to observance of the obligations regarding the processing of your personal data, to contact the Controller or The Office for Personal Data Protection,
  • the personal data are not subject to automated decision-making including profiling,
  • the Controller has no intention to forward the Subject’s data to third countries, international organizations or other parties not stated above; this consent, therefore, does not apply to a transfer of data to a foreign country,
  • the Controller did not appoint a data protection officer, did not authorize any personal data processor or appoint a representative to meet the obligations arising from the Regulation.

The Data Subject declares that they were duly informed about personal data processing and protection, that the above stated personal data are accurate and truthful, and they are provided to the Controller voluntarily and completely willingly.
